Appointment Deposit: A non-refundable initial deposit of $100 is required upon booking. By accepting this document, you acknowledge that you have read, understood, and agree to all terms outlined below.

Rescheduling: You may reschedule your appointment at any time, provided notification is given at least 48 hours prior to your scheduled appointment. To reschedule, please call 850-353-2564.

Reschedule Limit: You are permitted to reschedule your appointment up to three times. After the fourth rescheduling attempt, your appointment will be automatically canceled, and the initial deposit will be deemed non-refundable.

No-Show Policy: Failure to attend your scheduled appointment without prior notification (No-Show) will result in a $100 fee. This fee is non-refundable.

Medical Emergencies: If you are unable to attend your appointment due to a medical emergency, proper medical documentation must be provided to qualify for consideration.

Paperwork Requirements: Please be aware that paperwork must be completed and signed at the time of your appointment.
